How does this service work?

You provide us with the form type and content as well as information on how you would like your data saved and analyzed. We will design your form and provide you with a Word document, PDF version of the form, and your Remark Office OMR template. With this, you should be ready to go!

Form Design Service Steps:

  1. Fill out the Form Design Service Form Remark Products Group - Quote Request Form - You will be contacted to go over your order details.
  2. You provide the form content (email, Word doc, etc). Once the Form Data information has been reviewed and payment received, the Data Item Collection work on your form will begin.
  3. Expected turnaround time on the form and template, after Data Item Collection is complete, is 3 to 5 business days, depending on the length and complexity of the form.
Pricing starts at $500 per page for the form, form template creation, and one round of modifications. Additional modifications are $250 per revision. For forms greater than 3 pages, please contact us for pricing.
